Likely pathogenic for Citrullinemia — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_054012.4(ASS1):c.788T>A (p.Val263Glu), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with glutamic acid, which is acidic and polar, at codon 263 of the ASS1 protein (p.Val263Glu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical and/or biochemical features of citrullinemia (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt ASS1 protein function with a positive predictive value of 80%. This variant disrupts the p.Val263 amino acid residue in ASS1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 14680976, 18473344, 18925679, 23780642). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
