Pathogenic for Glutaric aciduria, type 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000159.4(GCDH):c.1146_1147dup (p.Arg383fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the GCDH gene (transcript NM_000159.4) at coding-DNA position 1146 through coding-DNA position 1147, duplicating 2 bases; at the protein level this means shifts the reading frame starting at arginine residue 383, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Arg383Profs*19) in the GCDH gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 56 amino acid(s) of the GCDH protein. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with GCDH-related conditions. This variant disrupts a region of the GCDH protein in which other variant(s) (p.Ala382Thr) have been determined to be pathogenic (PMID: 27629047, 33064266). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
