Uncertain significance for Hereditary breast ovarian cancer syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_007294.4(BRCA1):c.1938C>G (p.Ser646Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 1938, where C is replaced by G; at the protein level this means replaces serine at residue 646 with arginine — a missense variant. Submitter rationale: This sequence change replaces serine, which is neutral and polar, with arginine, which is basic and polar, at codon 646 of the BRCA1 protein (p.Ser646Arg). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BRCA1-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on BRCA1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
