This variant lies in the PKHD1 gene (transcript NM_138694.4) at coding-DNA position 3, where G is replaced by A; at the protein level this means replaces methionine at residue 1 with isoleucine — a missense variant. Submitter rationale: This sequence change affects the initiator methionine of the PKHD1 mRNA. The next in-frame methionine is located at codon 9. This variant is not present in population databases (gnomAD no frequency). Disruption of the initiator codon has been observed in individual(s) with autosomal recessive polycystic kidney disease (PMID: 27225849). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant disrupts a region of the PKHD1 protein in which other variant(s) (Leu8Pro) have been observed in individuals with PKHD1-related conditions (internal data). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
