Likely Pathogenic for GUCY2D-related recessive retinopathy — the classification assigned by ClinGen Leber Congenital Amaurosis/early Onset Retinal Dystrophy Variant Curation Expert Panel, ClinGen to NM_000180.4(GUCY2D):c.3078_3083dup (p.Leu1028_Arg1029insIleLeu), citing ClinGen LCAeoRD ACMG Specifications GUCY2D V1.0.0: NM_000180.4(GUCY2D):c.3078_3083dup (p.Ile1027_Leu1028dup) is a short in-frame insertion variant predicted to cause a change in the length of the protein by duplicating residues 1027 and 1028, with at least one of the neighboring base pairs highly conserved with a PhyloP conservation score of 7.80781 (PM4). This variant is absent from gnomAD v4.1.0 (PM2_Supporting). This variant has been reported in at least 1 proband with early-onset severe retinal dystrophy who was compound heterozygous with the NM_000180.4(GUCY2D):c.726del (p.Ile243SerfsTer3) variant confirmed in trans, which was previously classified pathogenic by the ClinGen LCA / eoRD VCEP (1 point, PMID: 27375279, PM3). A second proband has been identified as a compound heterozygote with the NM_000180.4(GUCY2D):c.937C>T (p.Arg313Cys) variant confirmed in trans (PMID: 34048777), but has not been curated by the LCA/eoRD VCEP and so is not counted for PM3 (1 total point, PM3) . At least one proband harboring this variant exhibits a phenotype including a diagnosis of LCA (0.5 points) with age of onset at 0.3 years (1 pt), genotyping by exome sequencing with no alternative cause of retinal disease identified (4 pts), poor vision (1 pt), roving nystagmus (1 pt), no light perception, attenuated vessels (0.5 pts), and extinguished electroretinogram responses from rods (0.5 pts) and cones (1 pt). Together these are specific for GUCY2D-related recessive retinopathy total of 9.5 points, PMID: 34048777, PP4_Moderate). In summary, this variant meets the criteria to be classified as likely pathogenic for GUCY2D-related recessive retinopathy based on the ACMG/AMP criteria applied, as specified by the ClinGen LCA / eoRD VCEP: PM2_Supporting, PM3, PM4, and PP4_Moderate.
