Pathogenic for Pontocerebellar hypoplasia type 1B — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_016042.4(EXOSC3):c.672_673del (p.Tyr225fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the EXOSC3 gene (transcript NM_016042.4) at coding-DNA position 672 through coding-DNA position 673, deleting 2 bases; at the protein level this means shifts the reading frame starting at tyrosine residue 225, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Tyr225Serfs*15) in the EXOSC3 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 51 amino acid(s) of the EXOSC3 protein. This variant is present in population databases (rs758812524, gnomAD 0.07%). This variant has not been reported in the literature in individuals affected with EXOSC3-related conditions. This variant disrupts a region of the EXOSC3 protein in which other variant(s) (p.Trp238Arg) have been determined to be pathogenic (PMID: 22544365). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
