Pathogenic for 46 XY differences of sex development; Oligosynaptic infertility — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_004959.5(NR5A1):c.247G>A (p.Val83Met), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine, which is neutral and non-polar, with methionine, which is neutral and non-polar, at codon 83 of the NR5A1 protein (p.Val83Met). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of 46,XY sex reversal (PMID: 30425642, 38128121). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt NR5A1 protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic.
