This variant lies in the ITGB4 gene (transcript NM_000213.5) at coding-DNA position 754, where C is replaced by T; at the protein level this means replaces arginine at residue 252 with cysteine — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 252 of the ITGB4 protein (p.Arg252Cys). This variant is present in population databases (rs201494421, gnomAD 0.004%). This missense change has been observed in individual(s) with autosomal recessive epidermolysis bullosa with or without pyloric atresia (PMID: 9792864, 14705814, 18779879). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on ITGB4 protein function. Studies have shown that this missense change alters ITGB4 gene expression (PMID: 14705814). For these reasons, this variant has been classified as Pathogenic.
