Pathogenic for Hereditary nonpolyposis colorectal neoplasms — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000249.4(MLH1):c.2030del (p.Ser677fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the MLH1 gene (transcript NM_000249.4) at coding-DNA position 2030, deleting one base; at the protein level this means shifts the reading frame starting at serine residue 677, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change results in a frameshift in the MLH1 gene (p.Ser677Ilefs*106). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 80 amino acid(s) of the MLH1 protein and extend the protein by 25 additional amino acid residues. This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with MLH1-related conditions. This variant disrupts a region of the MLH1 protein in which other variant(s) (p.Arg755Ser) have been determined to be pathogenic (PMID: 17510385, 20533529, 21952876; Invitae). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
