Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000038.6(APC):c.1313-1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the APC gene (transcript NM_000038.6) at the canonical splice acceptor site of the intron immediately before coding-DNA position 1313, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1313-1G>A intronic variant results from a G to A substitution one nucleotide upstream from coding exon 10 of the APC gene. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. The resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNA decay. RNA studies have demonstrated that this alteration results in a transcript predicted to lead to a protein with an in-frame deletion of 3 amino acids and insertion of 1 amino acid (predicted p.M438_A440delinsT); however, the exact functional effect of the altered amino acid sequence is unknown (Ambry internal data). Based on the available evidence, the clinical significance of this variant remains unclear.
