Submitter rationale: This missense variant replaces cysteine with serine at codon 95 in the LDLR type A repeat 2 of the LDLR protein. This variant is also known as p.Cys74Ser in the mature protein. This variant alters a conserved cysteine residue that is critical for proper protein folding and function (PMID: 2088165, 6091915, 15952897). Computational prediction tools indicate that this variant has a deleterious impact on protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in one individual affected with familial hypercholesterolemia (PMID: 28235710). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). A different nucleotide change causing the same amino acid change, c.283T>A p.Cys95Ser, is considered to be disease-causing (CliVar variation ID: 251110). Multiple different variants affecting the same codon: p.Cys95Phe, p.Cys95Tyr, p.Cys95Arg and p.Cys95Gly; are considered to be disease-causing (ClinVar variation ID: 251114, 251114, 251111, 251112), suggesting that cysteine at this position is important for LDLR protein function. Based on the available evidence, this variant is classified as Likely Pathogenic.
