Pathogenic for Arrhythmogenic right ventricular dysplasia 8 — the classification assigned by Victorian Clinical Genetics Services, Murdoch Childrens Research Institute to NM_004415.4(DSP):c.1755dup (p.His586fs), citing ACMG Guidelines, 2015. This variant lies in the DSP gene (transcript NM_004415.4) at coding-DNA position 1755, duplicating one base; at the protein level this means shifts the reading frame starting at histidine residue 586, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: Based on the classification scheme VCGS_Germline_v1.3.4, this variant is classified as Pathogenic. Following criteria are met: 0102 - Loss of function is a known mechanism of disease in this gene and is associated with arrhythmogenic right ventricular dysplasia 8 (ARVD; MIM#607450) and other DSP-related cardiac disorders. (I) 0108 - This gene is associated with both recessive and dominant disease. Variants in this gene are usually inherited in a dominant manner, however rare reports of recessive inheritance have resulted in a more severe cardiac phenotype (OMIM). (I) 0115 - Variants in this gene are known to have variable expressivity. Age-dependent penetrance and variable expressivity are well-described aspects of arrhythmogenic cardiomyopathy (PMID: 29062697). (I) 0201 - Variant is predicted to cause nonsense-mediated decay (NMD) and loss of protein (premature termination codon is located at least 54 nucleotides upstream of the final exon-exon junction). (SP) 0251 - This variant is heterozygous. (I) 0301 - Variant is absent from gnomAD (both v2 and v3). (SP) 0701 - Other null variants comparable to the one identified in this case have very strong previous evidence for pathogenicity. There are at least ten NMD-predicted variants that have been classified as likely pathogenic or pathogenic (DECIPHER). (SP) 0803 - This variant has limited previous evidence of pathogenicity in an unrelated individual. This variant has been reported in a family with a history of arrhythmogenic cardiomyopathy, with the proband specifically diagnosed with left-dominant arrhythmogenic cardiomyopathy (PMID: 19095136). (SP) 0901 - This variant has strong evidence for segregation with disease. This variant has been found to segregate in six affected members of the same family (PMID: 30382575). (SP) 1208 - Inheritance information for this variant is not currently available in this individual. (I) Legend: (SP) - Supporting pathogenic, (I) - Information, (SB) - Supporting benign
