Pathogenic for Mandibulofacial dysostosis-microcephaly syndrome — the classification assigned by Victorian Clinical Genetics Services, Murdoch Childrens Research Institute to NM_004247.4(EFTUD2):c.271+2T>A, citing ACMG Guidelines, 2015. This variant lies in the EFTUD2 gene (transcript NM_004247.4) at the canonical splice donor site of the intron immediately after coding-DNA position 271, where T is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: Based on the classification scheme VCGS_Germline_v1.3.4, this variant is classified as Pathogenic. Following criteria are met: 0102 - Loss of function is a known mechanism of disease in this gene and is associated with mandibulofacial dysostosis, Guion-Almeida type (MIM#610536). (I) 0107 - This gene is associated with autosomal dominant disease. (I) 0115 - Variants in this gene are known to have variable expressivity. Variable expressivity with variant transmission from parents with subclinical features has been reported (PMID: 33247512). (I) 0211 - Canonical splice site variant without proven consequence on splicing (no functional evidence available). (SP) 0251 - This variant is heterozygous. (I) 0301 - Variant is absent from gnomAD (both v2 and v3). (SP) 0505 - Abnormal splicing is predicted by in silico tools and affected nucleotide is highly conserved. (SP) 0703 - Other splice site variants comparable to the one identified in this case have moderate previous evidence for pathogenicity. c.271+1G>A was observed as de novo in an individual with mandibulofacial dysostosis, and a minigene assay showed the variant causes intron retention and an NMD-predicted frameshift (PMID: 32408545). c.271+3A>G has also been classified as likely pathogenic by a clinical laboratory in ClinVar. (SP) 0807 - This variant has no previous evidence of pathogenicity. (I) 0905 - No published segregation evidence has been identified for this variant. (I) 1007 - No published functional evidence has been identified for this variant. (I) 1203 - This variant has been shown to be de novo in the proband (parental status confirmed) (trio analysis).
