Likely pathogenic for Familial hypocalciuric hypercalcemia; Autosomal dominant hypocalcemia 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000388.4(CASR):c.1742C>T (p.Ala581Val), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 581 of the CASR protein (p.Ala581Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with hypocalciuric hypercalcemia (internal data). In at least one individual the variant was observed to be de novo. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
