Likely pathogenic for CYP21A2-related condition — the classification assigned by PreventionGenetics, part of Exact Sciences to NM_000500.9(CYP21A2):c.341C>A (p.Ser114Tyr). This variant lies in the CYP21A2 gene (transcript NM_000500.9) at coding-DNA position 341, where C is replaced by A; at the protein level this means replaces serine at residue 114 with tyrosine — a missense variant. Submitter rationale: The CYP21A2 c.341C>A variant is predicted to result in the amino acid substitution p.Ser114Tyr. Also known as S113Y in the literature, this variant has been reported with the well-known milder pathogenic variant c.844G>T (p.Val282Leu) (aka V281L) in an individual with autosomal recessive non-classic (NC) congenital adrenal hyperplasia (CAH) (Table S7 of New et al. 2013. PubMed ID: 23359698). Of note, a different substitution at the same codon (p.Ser114Phe) was also reported with the c.844G>T (p.Val282Leu) variant in an individual with NC-CAH; and the functional study showed that this variant significantly reduced the enzyme activity below 4% (reported as p.Ser113Phe in de Paula Michelatto et al. 2016. PubMed ID: 27721825). A structure–phenotype correlations of CYP21A2 variants study suggested that these two substitutions (p.Ser114Tyr and p.Ser114Phe) at codon 114 break the hydrogen bonds and result in steric clashes; and therefore they are likely milder alleles associated with NC-CAH (reported as S113Y and S113F in Haider et al. 2013. PubMed ID: 23359706). The c.341C>A (p.Ser114Tyr) variant has not been reported in a large population database (http://gnomad.broadinstitute.org), indicating this variant is rare. This variant falls within a highly paralogous region. Allele frequency data should be interpreted with caution. Taken together, this variant is interpreted as likely pathogenic.
