NM_001347721.2(DYRK1A):c.1190dup (p.Lys398fs) was classified as Pathogenic for Intellectual disability; Global developmental delay; Febrile seizure (within the age range of 3 months to 6 years); Simple febrile seizure; Atypical behavior; DYRK1A-related intellectual disability syndrome by HUSP Clinical Genetics Laboratory, Hospital Universitario San Pedro De Logroño (HUSP), citing ACMG Guidelines, 2015. This variant lies in the DYRK1A gene (transcript NM_001347721.2) at coding-DNA position 1190, duplicating one base; at the protein level this means shifts the reading frame starting at lysine residue 398, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The variant was detected in a 6-years-old girl with microcephaly, psychomotor and acquisition of language delay and febrile seizures. She presented the variant c.1217dupA in exon 9 of DYRK1A in heterozygosity (NM_001396.4). It results in a frameshift, a premature termination codon and, therefore, a truncated protein (p.Lys407GlufsTer16). This variant is not detected in general population and has not been reported in pathogenic data bases. A genetic study has been carried out in the parents and it is determined that none of them presents the variant, so it appears de novo in our patient. The loss of function of the protein predicted by in silico tools added to de-novo origin made us labeled the variant as pathogenic. Pathogenic variants in DYRK1A have been associated with Intellectual developmental disorder autosomal dominant 7 (OMIM: 614104). This clinical entity is characterized by severe primary microcephaly, intellectual disability with delay in language acquisition or absence of the same, febrile epipeptic seizures, intrauterine growth retardation, feeding difficulties at early childhood and behavioral problems. Some dysmorphic features are observed including micrognathia, thick lower lip, mild hypotelorism and hypoplastic earlobes. The inheritance pattern is autosomal dominant.
