Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000179.3(MSH6):c.3556+2dup, citing Ambry Variant Classification Scheme 2023: The c.3556+2dupT intronic variant, results from a duplication of two nucleotides at nucleotide position 3556 after intron 6 of the MSH6 gene. This variant has been identified in a proband whose Lynch syndrome-associated tumor demonstrated high microsatellite instability and loss of MSH6 expression by immunohistochemistry (Li S et al. J. Med. Genet. 2020 Jan;57:62-69; external laboratory communication). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data; external laboratory communication). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic.
