Likely pathogenic for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_004281.4(BAG3):c.1440_1450del (p.Arg480fs), citing Ambry Variant Classification Scheme 2023: The c.1440_1450del11 variant, located in coding exon 4 of the BAG3 gene, results from a deletion of 11 nucleotides at nucleotide positions 1440 to 1450, causing a translational frameshift with a predicted alternate stop codon (p.R480Sfs*12). This alteration occurs at the 3' terminus of theBAG3 gene, is not expected to trigger nonsense-mediated mRNA decay, and only impacts the last 16.7% of the protein. However, premature stop codons are typically deleterious in nature and the impacted region is critical for protein function (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the majority of available evidence to date, this variant is likely to be pathogenic.
