Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_002734.5(PRKAR1A):c.3G>A (p.Met1Ile), citing Ambry Variant Classification Scheme 2023: The p.M1? variant (also known as c.3G>A) is located in coding exon 1 of the PRKAR1A gene and results from a G to A substitution at nucleotide position 3. This alters the methionine residue at the initiation codon (ATG). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is well conserved in available vertebrate species. Sequence variations that modify the initiation codon are expected to result in either loss of translation initiation, N-terminal truncation, or cause a shift in the mRNA reading frame. Based on the supporting evidence, this alteration is likely pathogenic for Carney complex; however, the association of this alteration with acrodysostosis is unknown.
