This variant lies in the BMP4 gene (transcript NM_001202.6) at coding-DNA position 215, where A is replaced by G; at the protein level this means replaces glutamine at residue 72 with arginine — a missense variant. Submitter rationale: A variant of uncertain significance has been identified in the BMP4 gene. The Q72R variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. It is not observed at a significant frequency in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The Q72R variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. This substitution occurs at a position that is conserved in mammals; however, Arginine is observed at this position in evolution. In silico analysis is inconsistent in its predictions as to whether or not the Q72R variant is damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant.
