NM_000258.3(MYL3):c.2del (p.Met1fs) was classified as Uncertain Significance for Hypertrophic cardiomyopathy by All of Us Research Program, National Institutes of Health, citing ACMG Guidelines, 2015. This variant lies in the MYL3 gene (transcript NM_000258.3) at coding-DNA position 2, deleting one base; at the protein level this means shifts the reading frame starting at methionine residue 1, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This variant results in the loss of the translation initiator methionine at codon 1 of the MYL3 gene. This variant is expected to disrupt translation initiation and result in an absent or truncated protein product. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in one individual affected with hypertrophic cardiomyopathy (PMID: 33495597). This variant has been identified in 2/250586 chromosomes in the general population by the Genome Aggregation Database (gnomAD). Clinical relevance of loss-of-function MYL3 truncation variants in autosomal dominant cardiovascular disorders is not clearly established. The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
