Uncertain Significance for Arrhythmogenic right ventricular cardiomyopathy — the classification assigned by All of Us Research Program, National Institutes of Health to NM_001943.5(DSG2):c.863dup (p.Glu289fs), citing ACMG Guidelines, 2015. This variant lies in the DSG2 gene (transcript NM_001943.5) at coding-DNA position 863, duplicating one base; at the protein level this means shifts the reading frame starting at glutamic acid residue 289, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This variant inserts 1 nucleotide in exon 8 of the DSG2 gene, creating a frameshift and premature translation stop signal. This variant is expected to result in an absent or non-functional protein product. To our knowledge, this variant has not been reported in individuals affected with cardiovascular disorders in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Although there is a suspicion for a pathogenic role, clinical relevance of loss-of-function DSG2 truncation variants in autosomal dominant arrhythmogenic right ventricular cardiomyopathy is not yet clearly established. The available evidence is insufficient to determine the role of this variant in autosomal dominant disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
