Uncertain Significance for Malignant hyperthermia, susceptibility to, 1 — the classification assigned by All of Us Research Program, National Institutes of Health to NM_000540.3(RYR1):c.859G>C (p.Gly287Arg), citing ACMG Guidelines, 2015. This variant lies in the RYR1 gene (transcript NM_000540.3) at coding-DNA position 859, where G is replaced by C; at the protein level this means replaces glycine at residue 287 with arginine — a missense variant. Submitter rationale: This missense variant replaces glycine with arginine at codon 287 of the RYR1 protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.85, PMID: 27666373). This variant occurs in the N-terminal region (a.a. 1-552) of the RYR1 protein that is considered to be a hotspot for pathogenic variants that contribute to malignant hyperthermia susceptibility (PMID: 21118704). To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with RYR1-related disorders in the literature. This variant has been identified in 5/250304 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
