This variant lies in the SMAD4 gene (transcript NM_005359.6) at the canonical splice donor site of the intron immediately after coding-DNA position 1308 through 3 bases into the intron immediately after coding-DNA position 1308, inserting CTTTGATTTGCGTCAGTGTCATCGACAGATGCAGCAGCAGGCGGCTACTGCACAAGC. Submitter rationale: This variant is an insertion of 57 nucleotides between the +2/+3 positions in the splice donor region of intron 10 of the SMAD4 gene. Splice site prediction tools suggest that this variant may have a significant impact on RNA splicing, although this prediction has not been confirmed by RNA studies. This variant has not been reported in individuals affected with juvenile polyposis syndrome in the literature. This variant has been identified in 1/240648 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.
