Uncertain significance for Macrothrombocytopenia and granulocyte inclusions with or without nephritis or sensorineural hearing loss — the classification assigned by Molecular Genetics, Royal Melbourne Hospital to NM_002473.6(MYH9):c.4400C>T (p.Ala1467Val), citing ACMG Guidelines, 2015. This variant lies in the MYH9 gene (transcript NM_002473.6) at coding-DNA position 4400, where C is replaced by T; at the protein level this means replaces alanine at residue 1467 with valine — a missense variant. Submitter rationale: This sequence change in MYH9 is predicted to replace alanine with valine at codon 1467, p.(Ala1467Val). The alanine residue is highly conserved (100 vertebrates, UCSC), and is located in the myosin tail 1 domain. There is a moderate physicochemical difference between alanine and valine. The highest population minor allele frequency in gnomAD v2.1 is 0.002% (2/113,160 alleles) in the European (non-Finnish) population, which is lower than the credible allele frequency for dominant hearing loss genes (MYH9 included). To our knowledge, this variant has not been reported in the literature in any individuals with MYH9-related disease. Multiple lines of computational evidence have conflicting predictions for the missense substitution (4/6 algorithms predict deleterious). Based on the classification scheme RMH Modified ACMG Guidelines v1.5.1, this variant is classified as a VARIANT OF UNCERTAIN SIGNIFICANCE. Following criteria are met: PM2_Supporting.
