This variant lies in the GCK gene (transcript NM_000162.5) at coding-DNA position 1361, where C is replaced by T; at the protein level this means replaces alanine at residue 454 with valine — a missense variant. Submitter rationale: Variant summary: GCK c.1361C>T (p.Ala454Val) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 4.2e-06 in 239696 control chromosomes. c.1361C>T has been observed in individuals affected with Maturity-onset diabetes of the young (MODY) (Katashima_2021, Peixoto-Barbosa_2024). These data indicate that the variant is likely to be associated with disease. A different variant affecting the same codon has been classified as likely pathogenic by our lab (c.1361C>A, p.Ala454Glu), supporting the critical relevance of codon 454 to GCK protein function.
