NM_000181.4(GUSB):c.1800_1801del (p.Arg600fs) was classified as Likely pathogenic for Mucopolysaccharidosis type 7 by Molecular Genetics Department, Kulakov National Medical Research Center for Obstetrics, Gynecology and Perinatology, citing ACMG Guidelines, 2015. This variant lies in the GUSB gene (transcript NM_000181.4) at coding-DNA position 1800 through coding-DNA position 1801, deleting 2 bases; at the protein level this means shifts the reading frame starting at arginine residue 600, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: A previously undescribed nucleotide variant creates a frameshift p.Arg600SerfsTer5 in the GUSB gene. The variant was observed in compound heterozygous state (inherited from mother according to WES trio) with another likely pathogenic variant in an individual affected with hydrops fetalis. Homozygous and compound heterozygous variants are reported in patients with Mucopolysaccharidosis VII, 253220. The variant is not present in population database (gnomAD no frequency). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as likely pathogenic.
