Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000444.6(PHEX):c.250G>A (p.Ala84Thr), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces alanine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 84 of the PHEX protein (p.Ala84Thr). This variant is present in population databases (rs767642869, gnomAD 0.002%), including at least one homozygous and/or hemizygous individual. This missense change has been observed in individual(s) with X-Linked hypophosphatemia (Invitae). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PHEX protein function. This variant disrupts the p.Ala84 amino acid residue in PHEX. Other variant(s) that disrupt this residue have been observed in individuals with PHEX-related conditions (PMID: 25042154; Invitae), which suggests that this may be a clinically significant amino acid residue.
