Uncertain significance for Progressive familial heart block type IB — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_017636.4(TRPM4):c.115C>T (p.Arg39Cys), citing Invitae Variant Classification Sherloc (09022015): This variant is not present in population databases (gnomAD no frequency). This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 39 of the TRPM4 protein (p.Arg39Cys). This variant has not been reported in the literature in individuals affected with TRPM4-related conditions. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function output the following: SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0". The cysteine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function.
