Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000918.4(P4HB):c.1291G>A (p.Glu431Lys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 431 of the P4HB protein (p.Glu431Lys). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with P4HB-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt P4HB protein function with a positive predictive value of 80%. Experimental studies have shown that this missense change affects P4HB function (PMID: 25084354). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
