NM_001395413.1(POR):c.272C>T (p.Ala91Val) was classified as Uncertain significance for Congenital adrenal hyperplasia due to cytochrome P450 oxidoreductase deficiency by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the POR gene (transcript NM_001395413.1) at coding-DNA position 272, where C is replaced by T; at the protein level this means replaces alanine at residue 91 with valine — a missense variant. Submitter rationale: This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 94 of the POR protein (p.Ala94Val). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of POR deficiency (Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on POR protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
