Uncertain significance for Autosomal recessive limb-girdle muscular dystrophy type 2T; Muscular dystrophy-dystroglycanopathy (congenital with intellectual disability), type B14; Muscular dystrophy-dystroglycanopathy (congenital with brain and eye anomalies), type A14 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_021971.4(GMPPB):c.221G>C (p.Arg74Pro), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with proline, which is neutral and non-polar, at codon 74 of the GMPPB protein (p.Arg74Pro). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with GMPPB-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt GMPPB protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
