This variant lies in the GLB1 gene (transcript NM_000404.4) at coding-DNA position 443, where G is replaced by T; at the protein level this means replaces arginine at residue 148 with leucine — a missense variant. Submitter rationale: Variant summary: GLB1 c.443G>T (p.Arg148Leu) results in a non-conservative amino acid change located in the Glycoside hydrolase 35, catalytic domain (IPR031330) of the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant was absent in 249244 control chromosomes. To our knowledge, no occurrence of c.443G>T in individuals affected with Mucopolysaccharidosis Type IVB (Morquio Syndrome B) and no experimental evidence demonstrating its impact on protein function have been reported. Different variants affecting the same codon have been classified as pathogenic by our lab (c.442C>A, p.Arg148Ser; c.442C>T, p.Arg148Cys), supporting the critical relevance of codon 148 to GLB1 protein function.
