Uncertain significance for Pulmonary fibrosis and/or bone marrow failure, Telomere-related, 3; Dyskeratosis congenita, autosomal recessive 5 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001283009.2(RTEL1):c.2634C>G (p.Ile878Met), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the RTEL1 gene (transcript NM_001283009.2) at coding-DNA position 2634, where C is replaced by G; at the protein level this means replaces isoleucine at residue 878 with methionine — a missense variant. Submitter rationale: This sequence change replaces isoleucine, which is neutral and non-polar, with methionine, which is neutral and non-polar, at codon 878 of the RTEL1 protein (p.Ile878Met). This variant is present in population databases (rs778004637, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with RTEL1-related conditions. An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
