NM_000360.4(TH):c.1229G>C (p.Arg410Pro) was classified as Likely pathogenic for Autosomal recessive DOPA responsive dystonia by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TH gene (transcript NM_000360.4) at coding-DNA position 1229, where G is replaced by C; at the protein level this means replaces arginine at residue 410 with proline — a missense variant. Submitter rationale: This sequence change replaces arginine, which is basic and polar, with proline, which is neutral and non-polar, at codon 441 of the TH protein (p.Arg441Pro). This variant is present in population databases (rs367874223, gnomAD 0.0009%). This missense change has been observed in individuals with dystonia (PMID: 23939262, 27619486). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on TH protein function. Experimental studies have shown that this missense change affects TH function (PMID: 24753243). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
