Therefore, this variant has been classified as Likely Pathogenic. Experimental studies have shown that this missense change affects CYP11A1 function (PMID: 30620006). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt CYP11A1 protein function. This missense change has been observed in individual(s) with primary adrenal insufficiency (PMID: 30620006). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is present in population databases (rs535782968, gnomAD 0.01%). This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 460 of the CYP11A1 protein (p.Arg460Trp).
