Likely pathogenic for RYR1-related disorder — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000540.3(RYR1):c.5119C>T (p.Arg1707Cys), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with cysteine, which is neutral and slightly polar, at codon 1707 of the RYR1 protein (p.Arg1707Cys). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with autosomal recessive congenital muscular dystrophy (PMID: 38544359). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2898653). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt RYR1 protein function with a positive predictive value of 80%. This variant disrupts the p.Arg1707 amino acid residue in RYR1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 23919265; internal data). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
