Uncertain significance for Neuronal ceroid lipofuscinosis 1 — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000310.4(PPT1):c.491G>A (p.Arg164Gln), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 164 of the PPT1 protein (p.Arg164Gln). This variant is present in population databases (rs201892691, gnomAD 0.004%). This variant has not been reported in the literature in individuals affected with PPT1-related conditions. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PPT1 protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
