Likely pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000400.4(ERCC2):c.2125A>C (p.Thr709Pro), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces threonine, which is neutral and polar, with proline, which is neutral and non-polar, at codon 709 of the ERCC2 protein (p.Thr709Pro). This variant is present in population databases (rs758758729, gnomAD 0.006%). This missense change has been observed in individual(s) with clinical features of ERCC2-related disorders and/or clinical features of xeroderma pigmentosum (PMID: 33733458; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2894087). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt ERCC2 protein function with a negative predictive value of 80%. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic.
