Pathogenic for 3-Oxo-5 alpha-steroid delta 4-dehydrogenase deficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000348.4(SRD5A2):c.492_493insGA (p.Tyr165fs), citing Invitae Variant Classification Sherloc (09022015): This sequence change results in a frameshift in the SRD5A2 gene (p.Tyr165Aspfs*115). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 90 amino acid(s) of the SRD5A2 protein and extend the protein by 24 additional amino acid residues. This variant is present in population databases (no rsID available, gnomAD 0.001%). This variant has not been reported in the literature in individuals affected with SRD5A2-related conditions. This variant disrupts a region of the SRD5A2 protein in which other variant(s) (p.Arg246Gln) have been determined to be pathogenic (PMID: 1522235, 2665940, 10718838, 14594182, 18097518, 18717241, 19492581, 20190539, 20493473, 20583543, 23329752, 25899528, 26446026, 26980298). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic.
