Uncertain significance for Long QT syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000218.3(KCNQ1):c.104C>G (p.Pro35Arg), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the KCNQ1 gene (transcript NM_000218.3) at coding-DNA position 104, where C is replaced by G; at the protein level this means replaces proline at residue 35 with arginine — a missense variant. Submitter rationale: This variant is not present in population databases (gnomAD no frequency). This sequence change replaces proline, which is neutral and non-polar, with arginine, which is basic and polar, at codon 35 of the KCNQ1 protein (p.Pro35Arg). This variant has not been reported in the literature in individuals affected with KCNQ1-related conditions. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on KCNQ1 protein function.
