Uncertain significance — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_024120.5(NDUFAF5):c.326A>C (p.Lys109Thr), citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces lysine, which is basic and polar, with threonine, which is neutral and polar, at codon 109 of the NDUFAF5 protein (p.Lys109Thr). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with NDUFAF5-related conditions. ClinVar contains an entry for this variant (Variation ID: 2881980). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on NDUFAF5 protein function. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant disrupts the p.Lys109 amino acid residue in NDUFAF5. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 30473481). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing.
