Pathogenic for Autosomal dominant childhood-onset proximal spinal muscular atrophy with contractures — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_001003800.2(BICD2):c.1922T>A (p.Leu641Gln), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the BICD2 gene (transcript NM_001003800.2) at coding-DNA position 1922, where T is replaced by A; at the protein level this means replaces leucine at residue 641 with glutamine — a missense variant. Submitter rationale: This sequence change replaces leucine, which is neutral and non-polar, with glutamine, which is neutral and polar, at codon 641 of the BICD2 protein (p.Leu641Gln). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with clinical features of BICD2-related conditions (internal data). In at least one individual the variant was observed to be de novo. Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt BICD2 protein function with a positive predictive value of 80%. For these reasons, this variant has been classified as Pathogenic.
