Uncertain significance for Saldino-Mainzer syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_014714.4(IFT140):c.4207del (p.Arg1403fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the IFT140 gene (transcript NM_014714.4) at coding-DNA position 4207, deleting one base; at the protein level this means shifts the reading frame starting at arginine residue 1403, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This sequence change creates a premature translational stop signal (p.Arg1403Glyfs*13) in the IFT140 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 60 amino acid(s) of the IFT140 protein. This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individual(s) with clinical features of IFT140-related conditions and/or retinitis pigmentosa (PMID: 38531366; internal data). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 2858569). Experimental studies and prediction algorithms are not available or were not evaluated, and the functional significance of this variant is currently unknown. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
