This variant lies in the EYA4 gene (transcript NM_004100.5) at coding-DNA position 1776, duplicating one base; at the protein level this means shifts the reading frame starting at glycine residue 593, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This variant has not been reported in the literature in individuals affected with EYA4-related conditions. For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the EYA4 protein in which other variant(s) (p.Lys612*) have been determined to be pathogenic (PMID: 30123251). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant is not present in population databases (gnomAD no frequency). This sequence change results in a frameshift in the EYA4 gene (p.Gly593Trpfs*69). While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 47 amino acid(s) of the EYA4 protein and extend the protein by 21 additional amino acid residues.
