This variant lies in the ETHE1 gene (transcript NM_014297.5) at coding-DNA position 1, where A is replaced by C; at the protein level this means replaces methionine at residue 1 with leucine — a missense variant. Submitter rationale: Disruption of the initiator codon has been observed in individual(s) with ethylmalonic encephalopathy (PMID: 14732903, 25058219, 30298498, 32923369). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This sequence change affects the initiator methionine of the ETHE1 mRNA. The next in-frame methionine is located at codon 28. This variant is not present in population databases (gnomAD no frequency). Studies have shown that disruption of the initiator codon alters ETHE1 gene expression (PMID: 32923369). For these reasons, this variant has been classified as Pathogenic.
