Uncertain significance for Li-Fraumeni syndrome — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000546.6(TP53):c.993G>T (p.Gln331His), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the TP53 gene (transcript NM_000546.6) at coding-DNA position 993, where G is replaced by T; at the protein level this means replaces glutamine at residue 331 with histidine — a missense variant. Submitter rationale: In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. This variant disrupts the c.993G nucleotide in the TP53 gene. Other variant(s) that disrupt this nucleotide have been determined to be pathogenic (PMID: 22653678, 29324801, 33674644). This suggests that this nucleotide is clinically significant, and that variants that disrupt this position are likely to be disease-causing. Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. Experimental studies have shown that this missense change does not substantially affect TP53 function (PMID: 12826609, 19454241). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals affected with TP53-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 331 of the TP53 protein (p.Gln331His). This variant also falls at the last nucleotide of exon 9, which is part of the consensus splice site for this exon.
