Pathogenic — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000785.4(CYP27B1):c.1209del (p.Asn405fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the CYP27B1 gene (transcript NM_000785.4) at coding-DNA position 1209, deleting one base; at the protein level this means shifts the reading frame starting at asparagine residue 405, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the CYP27B1 protein in which other variant(s) (p.Arg492Trp) have been determined to be pathogenic (PMID: 22588163, 30282619). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with CYP27B1-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Asn405Ilefs*69) in the CYP27B1 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 104 amino acid(s) of the CYP27B1 protein.
