Pathogenic for Early-infantile DEE — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_172107.4(KCNQ2):c.2045_2046insT (p.Arg682fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the KCNQ2 gene (transcript NM_172107.4) at coding-DNA position 2045 through coding-DNA position 2046, inserting T; at the protein level this means shifts the reading frame starting at arginine residue 682, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: For these reasons, this variant has been classified as Pathogenic. This variant disrupts a region of the KCNQ2 protein in which other variant(s) (p.Lys829Serfs*35) have been determined to be pathogenic (Invitae). This suggests that this is a clinically significant region of the protein, and that variants that disrupt it are likely to be disease-causing. This variant has not been reported in the literature in individuals affected with KCNQ2-related conditions. This variant is not present in population databases (gnomAD no frequency). This sequence change creates a premature translational stop signal (p.Arg682Serfs*183) in the KCNQ2 gene. While this is not anticipated to result in nonsense mediated decay, it is expected to disrupt the last 191 amino acid(s) of the KCNQ2 protein.
