Pathogenic for X-linked severe combined immunodeficiency — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NM_000206.3(IL2RG):c.268_269del (p.Trp90fs), citing Invitae Variant Classification Sherloc (09022015). This variant lies in the IL2RG gene (transcript NM_000206.3) at coding-DNA position 268 through coding-DNA position 269, deleting 2 bases; at the protein level this means shifts the reading frame starting at tryptophan residue 90, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: This variant is not present in population databases (gnomAD no frequency). For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. This variant has not been reported in the literature in individuals affected with IL2RG-related conditions. This sequence change creates a premature translational stop signal (p.Trp90Valfs*6) in the IL2RG gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in IL2RG are known to be pathogenic (PMID: 9058718, 10794430).
